TELEMEDICINE SERVICES
Tallman Medical LLC may provide healthcare services through telemedicine platforms when clinically appropriate and legally permitted. Telemedicine has limitations and may not be suitable for all patients, conditions, or situations. Services are provided only in jurisdictions where the provider is appropriately licensed and authorized to practice.
APPOINTMENTS & PAYMENTS
Payment is required prior to scheduled appointments unless otherwise arranged. Appointments are not considered confirmed until payment has been received.
Fees for services are subject to change without notice. Any applicable refunds will be determined in accordance with Tallman Medical LLC policies.
CANCELLATION & NO-SHOW POLICY
Patients must provide at least forty-eight (48) hours' notice to cancel or reschedule an appointment.
• Appointments canceled less than 48 hours before the scheduled appointment time may be charged 50% of the scheduled service fee. • Missed appointments ("No-Shows") may be charged 50% of the scheduled service fee. • Patients may reschedule appointments up to two (2) times. Additional rescheduling requests may require a new appointment booking and payment.
MEDICAL SERVICES & PRESCRIPTIONS
Treatment recommendations, prescriptions, and healthcare services are provided only when medically appropriate following evaluation by a licensed healthcare provider. No guarantee is made regarding treatment outcomes or results.
